The following instance lacks a PROMPT attribute and so should only
show the default prompted generated by the browser:
There is not much to prompt, but a more sophisicated program could load up
responses one by one, sending an HTML file with a different Prompt for different
information until it has it all. As seen here, again each <ISINDEX>
widget does the same exact thing with the user input.
